  • Take 5 or more medications (polypharmacy)
  • Undergo chronic pain treatment
  • Receive psychiatric therapy
  • Experience signs of an ADE
  • Are unresponsive to therapy
  • Need a baseline assessment
  • Have recently been hospitalized
