The benefits of CV medication adherence testing have been reported in scientific literature. A recent study published by the journal Hypertension describes the patient health benefits.

Biochemical screening for nonadherence is associated with blood pressure reduction and improvement in adherence

Gupta study on hypertension

  • 238 patients in the UK with hypertension and suspected nonadherence to medication therapy underwent urinalysis to determine nonadherence with follow-up conversations between provider and patient regarding results.
  • Urinalysis included testing of commonly prescribed antihypertensive medications by liquid chromatography-tandem mass spectrometry (LC-MS-MS).
  • At baseline, 31% of patients were identified as nonadherent based on initial urinalysis; these patients had significantly higher systolic (SBP) and diastolic blood pressure (DBP) values than those identified as adherent.
  • 30 patients in the nonadherent group had repeat urinalysis performed along with measured BP values; of these patients, 80% demonstrated improved adherence and 53% became fully adherent based on urinalysis results.
  • Additionally, patients with follow-up in the nonadherent group experienced a significant drop in SBP and DBP of 19.5 and 7.5 mmHg, respectively, thus achieving similar blood pressure values to those who were persistently adherent throughout the study.

Important Conclusions/Application

  • Urinalysis to determine nonadherence to prescribed antihypertensive medications is associated with improved adherence, contributing to subsequent improvements in blood pressure.
  • Improving medication adherence in patients with hypertension can lead to reduced risk of coronary events, stroke, kidney disease and mortality.
  • Given the potential to improve adherence which can lead to reduced cardiovascular risk, urinalysis to determine nonadherence should be considered in patients with uncontrolled hypertension.
